Orthodontic Emergencies

An orthodontic emergency can feel alarming, but most issues that arise during braces or Invisalign treatment are minor and manageable at home until you can visit our office. At Dr. Boris Pinhasov’s Queens practice, we want every patient and parent to feel confident recognizing what needs same-day attention and what can safely wait for a scheduled appointment. Understanding the difference helps you stay calm, protect your appliances, and keep your treatment on track.
True Emergencies vs. Minor Issues
A true orthodontic emergency involves severe, uncontrollable pain, significant facial or oral swelling, trauma to the teeth or jaws, or an infection that is spreading. These situations require immediate professional care. If you experience any of these symptoms, call our office right away or go to the nearest emergency room. Dr. Pinhasov, a Board-Certified orthodontist and ABO Diplomate, coordinates closely with your general dentist and medical team to address emergencies that involve both your appliances and your overall oral health.
Minor issues are far more common. A loose bracket, a poking wire, or a displaced elastic band can be uncomfortable, but they rarely require same-day treatment. Most of these problems can be temporarily managed at home until your next regular adjustment visit.
How to Handle Common Problems at Home
Loose or broken bracket: If a bracket detaches from a tooth but is still attached to the archwire, leave it in place and cover it with orthodontic wax to prevent irritation. Avoid pulling it off the wire. If the bracket comes off completely, save it in a small bag and bring it to your next appointment.
Poking or protruding wire: Use a pencil eraser or a cotton swab to gently push the wire flat against the tooth. If you cannot reposition it, clip the end with clean nail clippers that have been sterilized with rubbing alcohol, then cover the area with wax. A poking wire is the most frequently reported issue among patients wearing traditional braces, and it is almost always a quick fix.
Broken or lost retainer: If your retainer cracks or no longer fits, contact our office as soon as possible. Teeth can shift quickly without retention, especially in the first year after active treatment. Do not attempt to bend a broken retainer back into shape.
Lost or damaged aligner: If you lose an Invisalign tray, move to the next tray in your sequence and call our office for guidance. If the next tray feels too tight, go back to the previous tray until Dr. Pinhasov can evaluate your progress and advise on the best next step.
Dislodged elastic ligature or loose molar band: A rubber ligature that slips off can be repositioned with clean tweezers. A loose molar band, however, should be evaluated at your next visit because it can affect how forces are applied to your teeth.
When to Call Our Office
Contact BP Smiles Orthodontics if you experience any of the following:
- Severe pain that does not respond to over-the-counter pain relief
- Swelling of the face, gums, or jaw
- A tooth that has been knocked loose or displaced by an injury
- An appliance that has broken in a way that causes cuts or sores inside the mouth
- Signs of infection such as fever, pus, or a foul taste
For non-urgent concerns – such as mild soreness after an adjustment, a slightly loose wire, or general discomfort from new elastics – you can manage symptoms at home and address them at your next scheduled visit.
Your Orthodontic Emergency Kit
Dr. Pinhasov recommends that every patient keep a small emergency kit at home and in their school bag or travel kit. A well-stocked kit includes orthodontic wax, a small mirror, a pair of clean tweezers, nail clippers (for wire trimming), over-the-counter pain relievers, a travel toothbrush, and a small container for any pieces that come loose. Having these items on hand means you can address most minor issues immediately, reducing stress and preventing further damage to your appliances.
